Trump mental acuity concerns are increasing, according to several new national surveys conducted in Washington and across the United States.

Multiple polls show a majority of Americans now question President Donald Trump’s mental sharpness as he continues his second term in the White House.

The findings come one year after Trump returned to office following the 2024 election, when age and cognitive health dominated the campaign debate involving former President Joe Biden.

Reuters-Ipsos poll shows majority call Trump “erratic”

A new survey from Reuters and Ipsos found:

  • 61% of Americans agree Trump has “become erratic with age”
  • 30% of Republicans share that view
  • 45% describe him as mentally sharp, down from 54% in 2023

The poll was released ahead of Trump’s address to United States Congress, where he delivered one of the longest speeches of his presidency.

Although the numbers do not match the levels seen during Biden’s presidency, they reflect a measurable shift in public perception.

CNN poll tracks decline in stamina and sharpness ratings

A separate survey from CNN recorded similar movement:

  • 46% believe Trump has the stamina and sharpness to serve effectively
  • That figure stood at 53% in late 2023

The drop suggests voters are reassessing leadership capacity as Trump approaches his 80s.

Washington Post-ABC-Ipsos survey shows majority doubt sharpness

A joint poll by The Washington Post, ABC News and Ipsos reported:

  • 56% say Trump lacks the mental sharpness required for office
  • 51% question his physical health
  • Doubt over mental sharpness increased 13 points since 2023

These findings place Trump near levels recorded by Biden earlier in his presidency, when voters also debated age and fitness.

Pew Research data shows declining confidence among Republicans

Research from Pew Research Center revealed:

  • 32% of Americans are “very confident” in Trump’s mental fitness, down from 39%
  • 66% of Republicans express strong confidence, down from 75%

The numbers indicate some erosion within Trump’s political base, even as overall Republican support remains solid.

Comparisons to Biden remain central to debate

Age and cognitive health played a decisive role in the 2024 election cycle.

Biden, who left the race at 81, faced persistent scrutiny over verbal slips and public appearances.

Trump, now 79, frequently highlights his cognitive test results and campaign stamina. However, public focus has shifted to:

  • Verbal mix-ups during speeches
  • Limited medical disclosures from the White House
  • Visible signs of fatigue at public events

Political analysts note that perceptions of mental fitness often track overall approval ratings.

As public approval shifts, confidence in leadership traits can move with it.

Why Trump mental acuity concerns matter politically

Presidential fitness affects:

  • Voter trust
  • Legislative negotiations in Congress
  • International diplomacy
  • Campaign messaging heading into midterm elections

The White House has not indicated any medical concerns. However, polling data shows the issue remains active in voter evaluation.

Trump remains the oldest president elected in US history. Now, like his predecessor, he faces growing public examination over age and leadership capacity.
